Fancy Pets Grooming boarding agreement

Owner agrees to pay the rate provided per day for boarding. Owner also agrees to pay all costs and charges associated with any special treatments or care to be provided for the animal during the pet’s boarding period. 

Owner further agrees to pay all veterinary costs that may be required in conjunction with the health of the pet. If pet becomes ill or if pet requires medical attention, Fancy Pets Grooming LLC, at its sole discretion, may engage the services of the owner’s veterinarian or administer medicine, which expenses shall be paid by owner. If owner’s veterinarian is unavailable and immediate attention is required Fancy Pets Grooming LLC will secure the services of a licensed veterinarian of its choice at owner's expense.

Waiver of Fancy Pets Grooming LLC liability pertaining to senior pets or pets in poor health

Fancy Pets Grooming LLC cannot be held liable for pet’s wellbeing in situations where pet is old or in questionable health.

Owner understands that Fancy Pets Grooming LLC has relied upon their representation that their dog is in good health and have not harmed or shown aggressive or threatening behavior towards any person or any other pet. Any pre-existing health concerns or known health conditions will be disclosed and discussed prior to drop off.

Owner hereby certifies that Owner has read and understands this agreement. Owner agrees that by signing this contract and leaving pet with Fancy Pets Grooming LLC that the information as provided about the pet is accurate.
